Hall usage is for GFD Members Only; and may only be used for events for their spouse, dependents and family, parents, and in-laws. The member hosting the event must be present during the entire event. 2. Usage will be granted on room availability and may be cancelled in the event of an emergency. 3. The hall usage form must be filled out and returned to the District 14 days prior to event. If everything is not turned in by this time, your event will be cancelled. The form will also have to be signed off by the Chiefs office. The following must also be turned in at that time. A. Certificate of Insurance for $1 million dollars naming the Greenlawn Fire District as additional insured. If an outside vendor is being used and will be onsite, they must also submit a Certificate of Insurance for $1 million dollars naming the Greenlawn Fire District as additional insured B. One check made payable to Greenlawn Fire District for $100.00 which will be cashed. This is the fee for hall usage C. One check made payable to Greenlawn Fire District for $500.00 which will be given back after the event if the room is left in the condition it was found and no damage occurred. If not, the money will be used to correct any deficiencies. D. TIPS certificate from bartender if liquor will be served. E. All events must end at 11:00 pm unless prior approval is obtained from the District F. The maximum number of people attending must not exceed 125. G. No smoking or vaping allowed anywhere in the building H. Any spills or accidental mishaps are to be cleaned up prior to leaving. I. All garbage containers in the hall and kitchen must be emptied in dumpster. J. Use of kitchen is permitted. It must be left in condition found. K. Vacuum and mop will be available for you to clean after the party. Restrooms outside the hall must also be cleaned up after party. L. Parking for guests must not interfere with members responding to calls. M. Guests of the event are to stay in the meeting area and that floor. They are not to wander around the other areas of the firehouse, including the apparatus bays, rec room, dispatch area. N. Dispatchers on duty are not there as your party host and are not permitted to assist with the event.* I have read the policy above and will comply
